Time and pitch alignment software for ADR/post-production

Which software applications does everybody use to speed up their ADR workflow, deal with difficult conforming situations, reconstitute terrible on-set sounding scenes and utilise a better sounding performance as well as for other general sync, pitch and time purposes?

Thanks for sharing Kim - in my experience, lots of students studying film are often confused as to what ADR is exactly so this helps clarify it!
